* Create appendix D - Known problems, group by package, list symptoms
of known problems and solutions (example is the libcurses.a symlink
in chapter 5, bash)
* Chapter 5, bash: add note that when error occurs regarding
install-info (on Debian systems for example) it can be safely
ignored.
